Director of Engineering & Public Works/City Engineer Engineering and Public Works Department City of National City From: Josh Johnson Senior Project Manager Project Professionals Corp Project: Cacho Recreation Center HVAC Replacement-Project- CIP No.: 20-07 Subject: Change Order No. Change Order No. 3 provides for: The removal of the additional asphalt roofing uncovered after removal of PVC roofing for framing. Proceed per RFI #4 and details provided by the Structural engineer to frame equipment pads on the top side of the sheathing anchors and install equipment curbs to the structural components. The addition of crickets to the top of edge of the platforms to flow water runoff around the new platforms is also included. The roofing is to be replaced with additional insulation board to areas where asphalt roofing was removed to bring elevations within compliance to remove ponding. The removal of an existing abandoned pad near 5-5. Supporting ing Information: During demolition of the existing TPO roof asphalt roofing was uncovered and needed t.o be removed to get to the structural framing. After removing the existing sleepers and sheeting to install the 4x8 members for the perimeter of the new HVAC curbs it was found that there was existing sheetrock attached to the 2x4 rafters. Installing the 4x8 members would penetrate the sheetrock and possibly create issues with fireproofing. To avoid this the structural engineer revised the drawings to have the additional members fit with the existing scenario. Additional demolition was needed to remove an existing pad/curb that was no longer needed with the new equipment layout that was not accounted for in the plans. Time Adjustment: rent: Two (2) additional working days are to be added to the Contract as the additional work affected the critical path of the approved schedule. CIP NO. 20-07 May 24, 2022 Contractor: Southwest Construction Services Inc. 11653 Riverside Drive, Ste, 153 Lakeside, CA 92040 This Change Order modifies Contractor's and the City's obligations and rights under the abovementioned contract. This change order is not effective until approved by the City. Time adjustment Two (2) additional working days are to be added to the Contract as the additional work did affect the critical path of the approved schedule. Cost of Change Order: The City and the Contractor agree that the Contractor shall proceed with the removal of the additional asphalt roofing uncovered after removal of PVC roofing. Proceed per RFI #4 and details provided by the Structural engineer to frame equipment pads on the top side of the sheathing anchors and install equipment curbs to the structural components, The addition of crickets to the top of edge of the platforms to flow water runoff around the new platforms is also included. An existing abandoned pad near 5-5 shall also be removed. The total amount for all labor, material, equipment and ail other associated work is 1 , 2L23. Recommended by Project Manager: osh Johnson, PPC Date: 6/10/22 By signing this Change Order the Contracto confirms that he/she is completely familiar with and has fairly negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. Ail of the terms and conditions of the original contract documents are incorporated herein.
