Reason/Purpose for this Change Order:
The item "Replace Existing Storm Drain Grate, Type 18- " was shown with a quantity of 3 EA t three
separate locations on the plans. Field meausrement of the exisint grates along National City Boulevard at
Stations 16+50 and 26+00 showed that the existing frame size was 24" and not 18" as indicated on the plans.
In addition, the existing frame at Station 26+00 was a double grate whereas the plans only showed replacing a
single grate. This Change order compensates the contractor for the upsizing of the 2 of the grates for a 4"
frame and one compelete 24" grate not originally called out on the plans.
The City of Pation l City and Contractor agree to the following change to the original al Contract:
Extra Work at Agreed Unit Price
In accordance with Section 4-1.0 , "Changes and Extra Work', of the Standard Specifications, Contractor to replace
existing skit, drain grates on National City Boulevard at STA 16+50 (lea) and 26+00 (2ea) with Type 24-12x grates in
accordance with Caltrans Standard Plan D77B.
Extra Work at Agreed Unit Price 3 EA $1,192.O01EA = $3,576.00 increase
A unit price of $1,192 per Type 24-12x grate replacement will constitute full and complete compensation, including all
markups, for removing and replacing the storm drain grates, including all material and incidentals.
Decrease Bid Items At Bid Unit Price
Bid Item 12 "Repalce Storm Drain Grate, Type 18-9x" 22ea (-67%) @ $900/EA = -$1,800(-67%)
There will be no adjustment for item No.12 "Repalce Storm Drain Grate, Type 18-9x" in accordance with Section 3-2.2.3
"Decreases of More than 25 Percent" of the Specifications by reason of this change order.
Total Decrease Bid Item at Bid Unit Price =-$1,800.00 decrease
This change order does not affect the controlling activity_ No time adjustment is warranted.

price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor,
supervision, overhead, fees, bonds, insurance, profit, etc, and that there will be no further compensation. AEI of the terms and conditions of
the original contract documents are incorporated herein.
