Subject: Change Order No. 4
Change Order No. 4 provides for:
The additional cost of demolition and removal of unforeseen concrete and wire mesh identified in the irrigation
lateral trench crossing 18th Street. Demolition includes the pumping out of ground water and removal and disposal
of the concrete and wire rnesh.
Payment for this change order will be force account.
Lipporting Information:
While trenching for the irrigation laterals across 18th Street at approximate station +13,00, the Contractor
identified concrete containing wire mesh, per RFI #18, beneath the asphalt as well as groundwater within the
trench. The Contractor was directed to pump out the groundwater, demo the existing concrete road and dispose
of the concrete and wire mesh.
Time Adjustment:
No additional working days are being provided for this change in the scope of work as it did not affect the critical
path of the approved schedule.
Cost Adjustment:
This change order includes the additional cost to pump out groundwater and demolish and dispose of the existing
concrete and wire mesh beneath the asphalt. The additional cost for this change in the scope of work has been
agreed upon at a lump sum price of $1,691.52. The cost for this change of work was determined by force account.
The pay method and amount have been reviewed by the Project Manager and are considered fair and reasonable. The
supporting documentation is on file with the project records.

This Change Order modifies Contractor's and the City's obligations and rights under the abovementioned
contract. This change order is not effective until approved by the City.
Time diusent:
Zero (0) additional working days are to be added to the Contract as the additional work did not affect
the critical path of the approved schedule.
Cost of Chace Order:
The City and the Contractor agreed that the Contractor proceeded under force account with pumping
groundwater out of the irrigation lateral trench near station 2+13.00 crossing 18th street, and

below the finished surface. The additional work included but is not limited to pumping water out of the
trench, demo of the concrete and wire mesh, and disposal of the concrete with wire mesh, The total
amount for all labor, material, equipment and all other associated work is 1,691.52.

By signing this Change Order the Con actor con ms that he/she is completely familiar with and has fairly
negotiated its price, terms and conditions. If this ange Order is approved, Contractor will provide all of the
equipment, materials and labor necessary to provide a complete work as described above at that the price
stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with
the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds,
Insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the
original contract documents are Incorporated herein, The signer for the Contractor confirms that he/she is
authorized to bind the Contractor to this Change Order.
