Change Order No. provides for: The additional cost to mill and pave to a depth of 3" over approximately 9925 SF in lieu of performing full -depth pavement removal and rFeplacement. Payment for this change order will be as agreed upon at a unit price per ton. Supporting information: While excavating for placement of the electrical crossings for the flashing beacons on 18th Street, the Contractor identified groundwater and unsuitable soil below the existing asphalt.. In order to avoid this issue while performing frill -depth removal and replacement of asphalt as per the original approved plans, the Project Manager suggested to mill and pave the existing asphalt. As this operation would provide significant savings by resulting in the deletion of line items regarding excavation and backfilling with base materials and filter fabric, the Project Manager directed the Contractor- to extend the paving limits to include more of the existing cracking in the asphalt along 18 ' Street. ,Time Adjustment: Thirty-one (31) additional working days are being added to the contract for the additional time necessary to coordinate and perform the change in scope. Cost d/ustrrrnt. The additional cost for this change in the scope of work has been agreed upon at unit price per ton. 181.73 tons of asphalt were placed at $250.88/ton for a total cost of 45,5 2.4 .. This change order is not effective until approved by the City. Time adjustrnent: Thirty-one 1 additional working days are to be added to the Contract. Cost of Change Order: The City and the Contractor agree that the Contractor shall proceed with milling and paving halt to asphalt a depth of 3" approximately 9925 SF in lieu of performing full -depth pavement removal and replacement. The total amount for all labor, material, equipment and all other associated work Is $45592.42, Recommended by Project Manager: 1 Date: 2/23/23 By signing this Change Order the Co r ctoi- co irms that he/she is completely familiar with and has fairly negotiated its price, terms and conditions. If thi -'Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to p ovide a complete work as described above at that the price stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc, and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein.
