NJ CHANGE ORDER MEMORANDUM October., 2022 To: Roberto Yano Director of Engineering & Public Works/City Engineer Engineering and Public Works Department: City of National City From: Josh Johnson Project Manager Project Professionals Corp Project: Paradise Creek Educational Park Extension CIS' No. 19-33 Subject: Change Order No. 1 Change Order- No. 1 provides for: The additional cost for demolition of an unforeseen abandoned sewer manhole. Demolition includes the removal of the manhole ring and lid, backfilling with aggregate base and placing a concrete cap. Payment for this .change order will be force account. Supporting information: While preparing subgrade for the concrete raised crosswalk improvement crossing Hoover Avenue, the Contractor identified an existing abandoned sewer manhole per l Fl at approximate Station 1+62. As the manhole would be located within the structural section, it was determined that the sewer manhole be demolished in order to install the new improvements. Time Adjustment: No additional working days are being provided for this change in the scope of work as it did not affect the critical path of the approved schedule. Cost Adjustment: The additional cost for this change in the scope of work has been agreed upon at a lump sum price of 1, 48.55. The cost for this change of work was determined by force account. The pay method and amount have been reviewed by the Project Manager and are considered fair and reasonable. The supporting documentation is on file with the project records. Ste. L San Diego, CA 92126 October 21, 2022 This Change Order modifies Contractor's and the City's obligations and rights under the abovementioned contract. This change order is not effective until approved by the City. Time adjustment Zero (0) additional working days are to be added to the Contract as the additional work did not affect thecritical path of the approved schedule. Cost of Change Order: The City and the Contractor agree that the Contractor shalt proceed under time and materials with the demolition of the unforeseen abandoned sewer manhole at approximate Station 1+ 2 within the raised crosswalk improvements crossing Hoover Avenue. The additional work includes but is not limited to removal of the manhole ring and lid, backfill with aggregate base, and pouring a concrete cap. The total amount for all labor, material, equipment and all other associated work is $1,64855. Recommended by Project Manager: tf By signing this Change Order the Contractair confirms that hefshe is completely familiar with and has fairly negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein.
