By signing this Charicle Order the Contractor confirms that he/she is completely familiarwith its terms and conditions and has fairly
negotiated its price terms and conditions. It this Change Order is approved, Contractor will provide all of the equipment materials and
labor necessary to provide a complete work as described above at that the price stated herein. Additionally. Contractor confer n- s that the
price is fair and complete and represents all corns associated with the work. including but not limited to all equipment, materials, labor
supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions

Per our discussion, We agree, and we will complete the 6 moisture test in the Meeting Room and Hallway Room for
$5O0 each -for a -total -of 3,000,00.

Hello Sean
As we discussed yesterday, we really need to identify if we have any moisture issues in the existing carpeted areas in the
Meeting Room and the Hallway Room. In order to do this, we will need to test the existing slab for the moisture
content. Due to the size of each area, this will require 4 tests in the Meeting Room and 2 tests in the Hallway for a total
of 6 tests.
We are a team player, and we understand that additional cost can have an impact on a project. We want to continue to
work together as a team on this project so I am proposing that instead of charging for 6 moisture tests, we will only
charge for 3 moisture tests. The cost is identified on the Bid Form (3 1200 -- 3600)
Please let me know if this is acceptable, as we currently have the Flooring Contractor tentatively scheduled to set up the
tests this Friday.
