Sup ortinq Information: City Engineering conference room needed additional electrical receptacles and communication lines for future use. The plotter receptacle was also having issues running the plotter and needed to be placed on a designated circuit from the panel instead of a shared circuit. Time A cis/ ustm e n t: Zero (0) additional working days are to be added to the Contract as the additional work did not affect the critical path of the approved schedule. Cost Adjustment: Cost was determined by force account analysis for a total of $4,631.03. This change order is not effective until approved by the City. Time adjustment: Zero (0) additional working days are to be added to the Contract as the additional work did not affect the critical path of the approved schedule. Cost of ha-ne Order: The City and the Contractor agree that the Contractor shall procure and install a new electrical run from panel E in the main electric room in the basement up to the conference room on first floor with (2) new emergency receptacles, (2) triple CAT6 drops in the conference room and create a designated 20A receptacle at plotter on first floor. The total amount for all labor, material, equipment and all other associated work is 4,6 1.0 . If this Charge Order is approved, Contractor- will provide all of the equipment, materials and labor necessary to provide a complete work as described above atthat the price stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein.
