Payment for this change order will be at a lump sum price identified.
Supporting Information:
The kitchen and serving area had missing tiles before the start of the project. This work was not identified on the
project plans and was required to be performed by the County Health Inspector before final inspection would be
allowed.ICCS was requested by the city to request a price from the contractor to replace the tiles.
Time Adjustment:
Two additional days were added to the contract time to coordinate and perform the work.
Cost Adjustment:,
The pay method and amount have been reviewed by ICCS and are considered fair and reasonable. The supporting
documentation is on file with the project records.

order to replace damaged and missing tile per the County Health code. This Charge Order modifies
Contractor's and the City's obligations and rights under the abovementioned contract. This change order
is not effective until approved by the City.
Time a lustrent:
One (2) working day is added to contract time for the time to coordinate and replace this scope of work
requested by the city.
Cost of Change Order:
There is a cost increase to the original contract amount. $4,843,33
