To: Martha Juarez Acting Director of Engineering & Public Works/City Engineer Engineering and Public Works Department City of National City From: Josh Johnson Project Manager Project Professionals Corp Project: Paradise Creek Educational Park Extension CI P No.: 1 - 3 Subject: Change Order No. 16 Choi ge Order No. 16 rovides for: The adjustments in contract unit price to line items 8, 18, and 19. Payment for this change order will be as agreed upon at a lump sum price. Supporting Information: The actual installed quantities for line items 8, 18, and 19 were less than 75% of the contract bid quantity. As per f the project special provisions, decreases of more than 25% to the quantity of a line item may section -�.�.� o r� result in an adjustment in the payment for the line item. For line item 8 curb ramps, the Contractor -' had bid the project expecting to install ped m raps in one day. I-lowever, the p plansonly called for 2 ped ramps to be installed, which resulted in inefficient labor. The adjustment in price for this line item was to compensate the Contractor for the inefficient labor costs. For line item 18,AC paving, the scope of the asphalt paving was revised as per change order #8 which resulted in a p � e decrease in the ACpaving required by the Contractor for this project. The reduced scope maintained the large same requirements for equipment and labor. The adjustment in price for this line item was to compensate the q Contractor for the equipment and labor with less tonnage being placed. For line item 19,class II aggregate base, the scope of the class II aggregate base was reduced as per change order #8 . The reduction in quantity resulted ire short loads being requested while maintaining the same costs for equipment ment and labor. The adjustment in price for this line item was to compensate the Contractor for higher unit pricing in materials and for equipment and labor costs for the short loads. Cost of Change Order: As per section 3-2.2.3 of the project special provisions, decreases of more than 25 percent to the quantity of a line tern may result in an adjustment in the payment for the line item as negotiated. The City and the Contractor agree to the adjustments in the contract unit price on the following line items. • Line item 8 will be adjusted from $4,000 to $4,500 per each ped ramp for a net increase of $1,000 • Line Item 18 will be adjusted from $360 to $373 per ton of asphalt for a net increase of $840.97 • Line Item 19 will be adjusted from $78.00 to $98.61 per ton of class II aggregate base for a net increase of $660,14 The total increase amount for all adjustments in contract unit pricing is $2,501.11. Recommended by Project Manager: Date: By signing this Change Order the Contractor confirms that lie/she is completely familiar with and has fairly negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Contractor confirms that the price is fair and complete and represents all costs associated the work, including but not limited to all equipment, materials labor,supervision,p with insurance, � overhead, fees, bonds, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein.
