Change Order No. 13 provides for:
Removing and reinstalling "no ped crossing signs" to correctly reflect the new crosswalk improvements layout,
installing a new pedestrian barricade and additional striping as per the revised details 21 and 41 on sheet C1.8.
Payment for this change order will be as agreed upon at a lump sum price,
Supporti Information:
During the punch walk on February 15, 2023, PPC identified an issue with the existing barricade installed on the
northwest corner of Hoover Ave. and 18th St. The existing barricade directed pedestrian traffic to the crosswalk
west of the project site at Coolidge Ave, and 18th St. With the newly installed improvements, PPC directed the
Contractor to install a barricade on the southwest corner to restrict pedestrian crossing as well as to reinstall the
"no ped crossing signs" to direct traffic to the newly installed crosswalk.
During the punch walk Kimley-Horn identified the need for additional striping along Hoover Ave. to designate
opposing traffic across 18th St. The revised striping plan was supplied to the Contractor along with punch list on
February23, 2023.
Time Adjustrent:
Zero (0) additional working days are to be added to the Contract.
Cost Adjustment:
The additional cost for this change in the scope of work has been agreed upon at a lump sum price of 2,333.10.
The cost for this change in the work accounts for all labor, equipment, materials and ail other work associated with
this change order.
The pay method and amount have been reviewed by the Project Manager and are considered fair and reasonable. The
supporting documentation is on file with the project records.

This Change Order modifies Contractor's and the City's obligations and rights under the abovernentioned
contract. This change order is not effective until approved by the City.
Time adjustment:
Zero (0) additional working days are to be added to the Contract.
Cost of Change Order:
The City and the Contractor agree that the Contractor shall proceed with removing and reinstalling ''no
ped crossing signs" to correctly reflect the new crosswalk improvements layout, installing a new
pedestrian barricade and additional striping as per the revised details 21 and 41 on sheet C1.8. The
additional work includes but is not limited to installing a new barricade on the southwest corner of
Hoover Ave. and 18th St., reinstalling "no ped crossing signs" on the northwest and southwest corners
to reflect new crosswalk improvements, and providing additional striping as per details on sheet C1.8.
The total amount for all labor, material, equipment and ail other associated work is 2,333.10.

By signing this Change Order the Co actor con rrns that he/she is completely familiar with and has fairly
negotiated its price, terms and conditions. if thi s hange Order is approved, Contractor will provide all of the
equipment, materials and labor necessary to provide a complete work as described above at that the price
stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with
the work, including but not limited to all equipment, niaterials, labor, supervision, overhead, fees, bonds,
insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the
original contract documents are incorporated herein. The signer for the Contractor confirms that he/she is
authorized to bind the Contractor to this Change Order.
