E Cer, `cPATIO[: CHANGE Ol DER MEMORANDUM August 9th, 2023 To: Martha Juare Acting Director of Engineering & PublicWors/City Engineer Engineering and Public Works Department City of National City From: ,Nosh Johnson Senior Project Manager Project Professionals Corp Project: Civic Center Basement Power Upgrade CIP o.: 20-03 Subject: Change Order No 8 Change Order No. 8 provides for: Proceeded under time and materials with circuit tracing and labeling of panels and devices within basement and throughout other panels in building. This change order covers all the remaining panels completed outside of change order ##4. Supporting ti'ng Information: During site investigation it was discovered that the existing control panels have not been updated in years and many were incorrect with existing conditions. To update the labeling, the Contractor must trace all existing wire connections to see what is fed or if it is still in use. The Contractor proceeded under time and materials for this work and this change order covered all the remaining panels outside of the work completed under change order Time Ad/ustmnt: Zero (0) additional working days are to be added to the Contract as the additional work did not affect the critical path of the approved schedule. Cost Adjustment: Cost was determined by force account analysis for a total of $3,, 3/13. 8. osh Johnson, PPC 8/11/23 roject ariager Date / CALIFORNIA 4' NATIONAL CITp Civic Center Basement Power Upgrade CHANGE ORDER NO. g lP NO. 0-03 August 9th, 2023 Contractor: National Electric Works 44/10 Rainer Ave, Sprite 101 San Diego, CA 92120 This Change Order modifies Contractor's and the City's obligations and rights under the abovementioned contract. This change order is not effective until approved by the City. Time adjustment: Zero (0) additional working days are to be added to the Contract as the additional work did not affect the critical path of the approved schedule. Cost of Change Order: The City and the Contractor agree that the Contractor proceeded under time and materials with circuit tracing and labeling of panels and devices within basement and throughout other panels in building. This change order covers all the remaining panels completed outside of change order #4 to complete the building. The total amount for all labor, material, equipment and all other associated work is $3,343 .7, Recommended by Project Manager: ash Johnson, PPC Date: 8/11/23 By signing this Change Order the Contractor'` confirms that he/she is completely familiar with and has fairly negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, Insurance, profit, etc. andthatthere will be no further compensation. Alt of the terms and conditions of the original contract documents are incorporated herein. The signer for the Contractor confirms that he/she is authorized to bind the Contractor to this Change Order.
